Patient Participation Groups (PPGs)

Patient population of the Camden Health Evolution (CHE)

Camden Health Evolution (CHE) is looking to enhance the relationship that we have with our community and ensure we offer services that meet patients' needs. We already have strong links to the Patient Participation Groups (PPGs) of our member practices, but would like to expand these to co-produce and improve our services.

Our community can help us in the following ways:

• Help us understand and shape the services that matter most to support healthier living
• Provide feedback on patient experience of Camden services and where Camden Health Evolution (CHE) can help improve these
• Support integration of services to provide care closer to home
• Help Camden Health Evolution (CHE) achieve its vision, mission and values

We do not have patient group meetings at present, but we will regularly review this.
